获得成绩单 from 记录管理

学生 currently attending 长滩 Unified School District, request records from the school of attendance.

An official copy of your transcript or immunization record can be requested in person or by mail from the 记录管理 Office. The 记录管理 Office does not accept or send transcripts by email or by fax. 

TO REQUEST TRANSCRIPTS OR IMMUNIZATION RECORDS IN PERSON

请, visit our office at 2201 E. 市场大街., 加州长滩90805. The office is open to the public from 7:00 a.m. 到下午3点.m. Monday through Friday.

We do not offer same-day service. Transcript requests submitted in person are processed in 1-3 days.

The cost of transcripts is $12.00. 面对面, acceptable forms of payment are Cash, 钱的订单, 收银员支票, 万事达卡, 签证, 发现, and most debit cards 与 万事达卡/签证 logo. If you are paying in cash please bring the exact dollar amount. We do not have change. 

TO REQUEST TRANSCRIPTS OR IMMUNIZATION RECORDS VIA MAIL

1. Fill out and complete an Official Transcript Request Form (see the Official Transcript Request Form link at the bottom of the page).
2. Include a copy of valid photo identification. The only forms of acceptable identification by the 长滩 Unified School District, 记录管理 Office are; any state Department of Motor Vehicles Identification Card, 驾照, 领事我.D.，或护照.
   
   If you do not have a valid state-issued ID, send a copy of any of your 长滩 Unified School District Student I.D. 与 a copy of your birth certificate.
3. Include the total dollar amount of the requested services. Mailed requests are payable by Money Order/Cashier’s Checks only. 钱的订单 & 收银员支票 must be payable in U.S. 基金. Make payment payable to 菠菜网lol正规平台.
   
   Via mail personal checks, credit cards, or debit cards are not accepted.
4. Mail your request to the 记录管理 Office 与 all the required information.

请, allow 10 business days to process High School 成绩单 from the date your request is received by the 记录管理 Office. Requests are processed on a first-come first-served basis. Transcript requests are processed and mailed via regular United States Postal Service.

The 记录管理 Office does not verify receipt of request.

Requests will not be processed 与out the required information, proper identification, and fee.
